INSTRUCTIONS:

1. Payment must be made ONLINE before or on August 1^st, 2010 for the team to be entered into the tournament.
2. All team members will be required to sign the waiver for submission the morning of the tournament before they will be allowed to play. All team members should bring an appropriate age identification card with them to the tournament.
3. All team members under the age of 18 must have their parent sign a release form (click here to print waiver [pdf]). Any team member under 18 must bring this signed form with them on the day of the event and submit it to the registration area, before they will be allowed to play. All team members should bring an appropriate age identification card with them to the tournament.
